Cheek contouring and shaping is one of the most requested hyaluronic acid filler treatments. The reason is simple – the middle third of the face plays a key role in maintaining youthful proportions. High, softly rounded cheeks create the so-called “triangle of youth” – wider in the upper part and narrower in the lower third of the face. Over time, volume is lost in the cheeks, tissue descends and facial contours change. The face may look more tired, without clear structure and without the reflection of light that gives freshness. Hyaluronic acid fillers for the cheeks work by restoring lost volume in precisely defined anatomical points. When applied into deeper layers, often using a technique in which the product is placed on the bone, the treatment can achieve:

Problematic acne-prone skin can significantly affect self-confidence, and even when acne is successfully treated, acne scars may remain. Scars that are wide enough can be significantly improved by filling them with hyaluronic acid fillers. In our experience, the ideal combination of treatments for this problem includes CO2 laser, subcision, TCA peel and hyaluronic acid fillers.

We have all seen overfilled faces, oversized cheeks, unnatural movement of hyaluronic acid during facial expressions and smiling, elongated chins and an artificial look, so we completely understand your concern. Exaggeration and an unnatural appearance occur because of incorrect indication and because hyaluronic acid fillers are injected not only in excessive amounts, but also in the wrong place. Beauty standards change under the influence of social media, the cosmetics industry, fashion and the beauty industry. However, in order to achieve the desired treatment goal, we must first assess the natural characteristics of your face and not allow a current, often passing trend to disturb our judgement.

For a good result, a detailed facial examination is essential, first at rest and then in movement, so we can understand what happens to the face during speech and smiling and see how the treated area will behave during facial expressions. During the examination, it is necessary to assess the entire face, mark areas that do not require correction, areas where correction is necessary, as well as areas where changes are present but not overly pronounced.

The first step is treating the areas with the most pronounced changes, and this is also the first part of the treatment. After the first part of the treatment is completed, we schedule a follow-up appointment one month later, where we assess the results, see what has happened with the remaining changes, determine which of them require further correction and which do not, and create a further treatment plan.

The treatment begins with a doctor’s examination, during which the appropriate amount and type of filler are selected. After the examination, a local anaesthetic cream is applied to make the treatment comfortable. The next step is injection of hyaluronic acid with very fine needles or a blunt cannula. After the treatment, you are ready to return to your everyday activities.

For 24 hours after the treatment, makeup and cosmetic products should not be used. You should also avoid exercise or more intense physical activity. After that period, you can continue your usual facial skincare routine.

The treated area should not be massaged unless your doctor specifically recommends it. For the next 2 weeks, active sun exposure, sauna and other aesthetic or cosmetic treatments are not recommended.

The treated area may be cooled, but only briefly and not with ice in direct contact with the skin, in order to avoid frostbite. If you are prone to herpes simplex on the lips, inform your doctor so that appropriate measures can be used to prevent this infection.

The amount of hyaluronic acid is individual and depends on the treated area and the intensity of the changes being corrected. The only reliable way to determine this is an examination, where we create an individual plan based on age-related changes and the concerns you want to correct. To give you a general idea, here are approximate amounts of hyaluronic acid fillers for the most common corrections.

For cheek correction, the average amount needed for a visible effect is 1 ml of hyaluronic acid filler per side, or 2 ml for both cheeks.
When it comes to the under-eye area, the average amount for correction is 2 ml of hyaluronic acid, one of which is applied to the area around the tear troughs to provide support, and one directly into the tear troughs.
For filling nasolabial folds, the amount of hyaluronic acid filler ranges between 1 and 2 ml.
For lip correction, the amount of hyaluronic acid is usually 0.5–1 ml.
For shaping the jawline and chin, the amount usually needed is 3 ml.

Hyaluronic acid is a natural substance, which is why treatment with hyaluronic acid is very safe and adverse reactions are kept to a minimum. However, for the treatment to truly be safe, it is necessary to use registered and approved hyaluronic acid fillers. In a market with many hyaluronic acid fillers, a large number of which are of questionable quality, unwanted effects can easily occur.

It is always safest to use hyaluronic acid fillers approved by the U.S. Food and Drug Administration (FDA), which have passed strict control and met high standards. Unlike products with a European CE certificate, where only the content and correctness of the hyaluronic acid in the syringe are checked, FDA-approved hyaluronic acid fillers are also assessed for their behaviour in tissue after injection and over a longer period of time, which is crucial.

One of the best things about this procedure is that the results are visible immediately after the treatment, which lasts around fifteen minutes. As soon as your doctor gives you the mirror, you will be able to notice the difference, while the best aesthetic result is visible after two weeks, when the hyaluronic acid integrates nicely into your tissue and begins to “work”.

Hyaluronic acid fillers do not disappear all at once after the stated period of time, but gradually, which is why it is wise to plan a minimal correction after a certain period. For lips, this is usually after 6 months, while for wrinkles, under-eye areas and cheeks, it is usually after 6–12 months.

Cannula application and needle application each have their advantages and disadvantages.

1. By applying hyaluronic acid fillers with a cannula, tissue trauma is reduced because the procedure is performed through a single entry point. After cannula application, the likelihood of bruising and swelling is minimal, so there is no recovery period, which makes the cannula technique the method of choice for delicate areas such as the under-eye region. After hyaluronic acid filler treatment with a cannula, patients can immediately return to their usual activities, without bruising, swelling or downtime.

2. The advantages of the needle are greater precision and a stronger lifting effect. With a needle, hyaluronic acid fillers can be applied directly onto the bone, which provides the greatest lifting effect. The disadvantage of the needle technique is the possibility of small bruises.

The appearance of lumps is not a serious complication, but it should still be monitored.

After the treatment, the area where the hyaluronic acid filler was applied is gently massaged so that the lumps soften and the material is distributed more evenly. Sometimes a mild massage at home may be recommended if lumps are felt 48 hours after the procedure.
If the lumps do not disappear after 3–4 weeks (the time needed for the filler to “settle”), consult your doctor, who can remove them with massage or hyaluronidase.
Lumps that appear directly at the injection site are usually the result of poor injection technique and/or tissue sensitivity – so-called nodules – and most often disappear within 2–3 weeks.

It happens that patients come to us after having had the procedure elsewhere and are dissatisfied with the results because too much hyaluronic acid filler was applied, leaving them with the opposite of a rejuvenated and more attractive appearance. We solve this problem simply, thanks to the enzyme hyaluronidase, which is used to remove hyaluronic acid, together with the expertise and experience of our doctors.

The results of hyaluronidase, i.e. removal of hyaluronic acid filler, are visible within 15 minutes to 24–48 hours.

MD Codes (medical codes) are a technique for injecting hyaluronic acid fillers in which universal points on the face are selected for correction based on the problem. For example, according to this method, correction of nasolabial folds or descent of the midface requires correction of the cheeks, anterior cheek, the area beneath the cheeks and the folds themselves. But what happens if the patient already has prominent cheeks? You can conclude that this rule cannot be applied to everyone, because it can certainly lead to an unnatural appearance.

If we think about the above, we can conclude that even 1 ml of hyaluronic acid applied in the wrong place can be too much. Imagine a situation we often encounter in practice. A patient has pronounced nasolabial folds, volume loss in the front part of the face and the temple (temporal) region, but excellent projection of the outer cheek area. Cheek correction with just 1 ml of hyaluronic acid in that case can significantly worsen the facial appearance and further emphasise the areas where volume is missing. However, if we choose to correct only the areas where there is a problem – in this case the anterior cheek, temples and folds with 3 ml of hyaluronic acid – the patient will look completely natural and significantly younger.

Another problem that leads to an unnatural appearance is trying to solve a surgical problem with hyaluronic acid fillers. The essence is simple – if there is significant facial descent, excess skin and laxity of the subcutaneous tissue, the best result is achieved with surgical techniques. Excessive use of hyaluronic acid can mask these changes, but at the cost of an unnatural look, reflected in oversized cheeks, swollen-looking cheeks and an elongated chin, which we have all had the opportunity to see.

In this situation, there are two options: choose surgery for the optimal effect, or, if for any reason you are not ready for surgery, correct the changes with hyaluronic acid without overfilling the face or changing its physiognomy. It is true that age-related changes will still be visible, but they will be softened while preserving facial identity and a natural appearance.

Even after the filler has disappeared, skin treated with hyaluronic acid looks better and younger, because over time hyaluronic acid has stimulated skin renewal, hydrated the skin and provided support. You can stop using hyaluronic acid fillers at any time without consequences; on the contrary, the skin may look better even after complete resorption of the hyaluronic acid. That is why hyaluronic acid is the number one treatment in anti-ageing medicine.

If the treated area were aggressively pressed or massaged, there is a possibility that the filler could move. Therefore, it is advised not to touch the area unless you feel lumps – in that case, gentle massage may be applied.

In any case, if a hyaluronic acid filler is properly selected and injected, it does not migrate or “move around the face”, unlike silicone and similar fillers, which are dangerous and banned in most countries.

In most cases, there is no problem if there were no complications with the previous treatment. You must tell your doctor when you had the previous treatment, which product was used and in what amount.

Although this is very rare, it is still possible. That is why it is very important to have this procedure performed in a healthcare institution and by qualified doctors.

There are many different types of dermal fillers, but the most popular and most commonly used are hyaluronic acid fillers. They are used because of their safety, effectiveness and simple application. If bruises appear, some time is needed for them to fade, so it is important to plan the treatment in advance before any important event. Asymmetry can be corrected by adding a certain amount of dermal filler.

During and after the treatment, the treated area is gently massaged so that the applied material is distributed as evenly as possible and the appearance of small lumps is reduced. If lumps do not disappear one month after the treatment, they can be dissolved with the enzyme hyaluronidase.

Serious complications are very rare.

Very important note: A side effect of unverified fillers can be complications in the form of granulomas, which occur with poor-quality hyaluronic acid fillers or artificial materials.

Hyaluronic acid fillers are contraindicated in cases of severe somatic symptom disorders, acute phases of chronic diseases, local inflammation, dermatological disease in the injection area, immune system dysfunction, tendency to scar formation, infectious diseases and pregnancy.
