Why does the skin on the eyelids sag?

A youthful and radiant face with toned and glowing facial skin is something many of us desire. With proper care, we can strive to maintain a youthful appearance for as long as possible. However, the effects of aging will inevitably start to show on our faces, such as wrinkles, dry skin, and dark circles.
The skin around the eyes is particularly delicate and sensitive, making any changes in this area more noticeable. The first signs of aging, like smile lines, may appear around the eyes as early as the age of 25. As time goes by, the loss of subcutaneous tissue can lead to the formation of dark circles and sunken areas. Some individuals may also experience eye area swelling, the formation of bags, and swollen eyelids.
Excess skin on the eyelids, known as dermatochalasis, primarily appears due to the weakening of the eyelid tissue’s elasticity. While it is more commonly associated with aging, there are no strict rules. Even individuals in their early thirties can notice sagging skin on the eyelids, and the causes can vary.

Factors contributing to sagging eyelid skin include:

  1. Aging: The natural aging process leads to a decline in the production of collagen, elastin, and hyaluronic acid after the age of 25. The skin loses its elasticity, firmness, and volume. Weakening of the eyelid tissue combined with gravity can result in skin sagging.
  2. Genetics: Genetic factors can predispose some individuals to experience signs of aging earlier.
  3. Harmful UV Radiation: Exposure to harmful UV radiation is one of the main contributors to premature skin aging, wrinkles, and sagging. The thin skin around the eyes is particularly sensitive, and individuals with heightened facial photosensitivity are at a higher risk.
  4. Certain Diseases: Eye injuries and trauma can accelerate the loosening of skin on the eyelids. Excess eyelid skin can also be associated with long-term liver or kidney diseases, as well as an overactive thyroid gland. If you suspect a medical condition is causing eyelid skin laxity, it is important to consult a doctor.
  5. Lifestyle: Factors such as stress, fatigue, lack of sleep, smoking, alcohol consumption, and an unhealthy diet directly affect the skin’s appearance and can make it look older.

Droopy Eyelids

Drooping eyelids, particularly the upper eyelids, can pose not only an aesthetic concern but also functional and health problems. The presence of excess skin on the upper eyelids can lead to a narrowing of the field of vision. Patients may compensate for this by raising their eyebrows, which can contribute to the deepening of forehead wrinkles. Additionally, the excess skin may push the eyelashes towards the eye, leading to increased tearing and various eye irritations.

There are three main types of drooping eyelids:

  • Dermatochalasis: This type of drooping eyelids is primarily caused by the loss of tone and elasticity of the skin. Excess skin can be present on both the upper and lower eyelids, and it is most commonly associated with the natural aging process.
  • Blepharochalasis: The relaxation of the eyelid skin in this condition is often a result of frequent inflammation and swelling of the eyes. This leads to stretching, thinning, and atrophy of the skin in the eyelid area.
  • Ptosis of the eyelids: Ptosis refers to the drooping or sagging of the eyelids due to various factors such as optic nerve injury, eye trauma, stroke, or other underlying medical conditions.

When is it time for laser eyelid tightening?

When we are dissatisfied with our appearance and bothered by loose skin on the eyelids, which affects our self-confidence, it is time to take action. Shallow and expression lines around the eyes can be effectively eliminated using Botox. On the other hand, the best solution for filling sunken dark circles is hyaluronic fillers.
But how can we successfully address excess skin on the eyelids? For a long time, surgical eyelid tightening was the only option, but many patients opted out due to the high cost and potential risks involved. Laser eyelid tightening with the fractional Candela CO2 laser is the only effective non-surgical solution for removing excess skin on the eyelids.
Before looking into how this laser works and the results it achieves, we would like to share the wonderful experience of our patient, Daniela. She chose to address the issue of excess eyelid skin using this method. As a woman in her early forties, Daniela sought an anti-aging treatment that would regenerate her skin, slow down the aging process, and eliminate minor excess skin around her eyes. We successfully achieved that! Daniela’s before-and-after photos speak volumes, and you can view them here. Additionally, you can also see the experience of our patient, Biljana, with eyelid tightening using the CO2 laser here.
Now that you have seen real-life examples, let’s proceed.

Candela CO2 laser for tightening the lower eyelids

The CO2 laser is an essential tool in aesthetic and anti-aging medicine. It offers skin rejuvenation and effectively addresses issues such as scars, stretch marks, and hyperpigmentation. It targets wrinkles, restores skin elasticity, and delivers excellent results for the removal of excess skin on the lower eyelids. Due to the thin and sensitive nature of eyelid skin, a less invasive approach is required compared to other facial areas. The Candela CO2 laser is gentle on the patient’s skin, ensuring a safe and comfortable treatment experience.

Here are the three key advantages of the CO2 laser:

  1. Minimally invasive procedure.
  2. Long-lasting results.
  3. Shorter recovery period.

When discussing eyelid tightening, it is important to highlight that the CO2 laser is highly effective and the preferred solution for addressing mild to moderate excess skin on the lower eyelids. Smaller amounts of excess skin on the upper eyelids can also be treated with the laser. However, for larger amounts of excess skin, surgical intervention remains necessary.

How does the CO2 laser tighten the eyelids?

The Candela CO2 laser utilizes a dual laser wave that targets both the surface layer (epidermis) and the deeper layer (dermis) of the skin simultaneously. The laser beam generates controlled heat in the deeper skin layers, triggering the production of collagen and elastin. This process leads to tighter and more resilient skin. One of the key advantages of laser eyelid tightening is the shorter recovery period, while the results are long-lasting.
The CO2 laser treatment starts with an ablative procedure, which involves removing a thin, superficial layer of damaged skin. Subsequently, the laser creates microchannels in the deeper skin layers, applying heat to stimulate collagen production and promote the growth of new, healthy skin cells. The laser specifically targets the damaged skin, leaving the surrounding healthy tissue unaffected. This targeted approach enables faster healing and significantly reduces the recovery time.

Laser eyelid tightening – treatment, product, recovery?

Everything begins with a consultation with the patient and an examination conducted by our doctors. Once the patient communicates their expectations, a treatment plan is formulated in collaboration with the doctor.
The procedure commences with cleansing the skin, followed by the application of an anesthetic cream that takes effect for approximately one hour. Subsequently, the CO2 laser treatment, lasting approximately 20-30 minutes, is initiated. The treatment itself is nearly painless, with patients experiencing only a mild tingling sensation.
After the treatment, the skin may appear red and slightly swollen. The doctor will provide recommendations for proper skincare, including the mandatory application of a regenerative cream and the avoidance of intense sunlight. As a result, these treatments are typically performed from autumn to early spring. It is important to adhere to the guidelines provided by the doctor after laser eyelid tightening to ensure a smooth and swift recovery process.

How to prepare for the treatment?

Every laser treatment necessitates specific product. Refrain from using photosensitizing medications for at least 72 hours prior to the procedure. Four weeks before the treatment, avoid deep peels, chemical peels of the face, and prolonged sun exposure. Inform your doctor if you are undergoing any other therapies.

What is the duration of skin recovery?

On the first day, the eyelid skin may appear swollen, red, and tight. The swelling usually subsides by the second day. On the third day, the skin may take on a brownish hue and begin to peel, a process that generally concludes by the fifth day. The skin may remain slightly red for an additional day or two, and makeup can be used to conceal it.

How many treatments are required?

Typically, two to four treatments are necessary, depending on the skin condition and the extent of excess skin on the eyelids. There should be a gap of one and a half to two months between each treatment. Thus, commencing the treatments at the start of autumn is advisable, as it allows the skin on the eyelids to be tightened and rejuvenated by spring. Noticeable improvements can be observed after the first treatment, and greater enhancements are usually noticed following the second treatment. Over the subsequent months, healthy, youthful skin cells are generated, while collagen and elastin synthesis significantly improves the quality of the skin around the eyes. The skin becomes regenerated, tightened, and radiant.

Laser eyelid tightening – effects and results?

The CO2 laser is highly effective in addressing mild to moderate excess skin on the eyelids, offering the following benefits:

  • Gentle tightening of the eyelid skin
  • Stimulation of collagen synthesis, leading to skin rejuvenation
  • Reduction of wrinkles, particularly under the eyes

Contraindications

Pregnancy, breastfeeding, and certain skin conditions such as rosacea, psoriasis, dermatitis, and acne, as well as malignant diseases, are considered contraindications for laser skin treatments. It is important to consult with a healthcare professional or dermatologist to determine if laser treatment is suitable for your specific situation, taking into account any contraindications and potential risks.

Is laser tightening of the upper eyelids possible?

We can only apply laser tightening for slight excess skin on the upper eyelids, but it is important to have realistic expectations. Achieving satisfactory results with laser methods for the upper eyelids is more challenging compared to the lower eyelids. Typically, an improvement of around 20 percent can be expected for the upper eyelids.
Unfortunately, despite the advancements in aesthetic medicine, there is currently no effective non-surgical solution for significant excess skin on the upper eyelids. According to Dr. Miloš Živković, our doctor, the only long-term solution for excess skin on the upper eyelids is blepharoplasty, which is a surgical procedure. Blepharoplasty involves removing excess skin and sometimes a portion of fatty tissue and muscle on the upper eyelids.

How to improve the appearance of the skin around the eyes?

Proper care and suitable treatments are essential for maintaining a tight and youthful appearance of the skin around the eyes. We recommend several treatments that can help improve the skin’s appearance:

  • Biorevitalization: This treatment involves the use of hyaluronic acid or other bioactive substances to hydrate and rejuvenate the skin. It helps to improve skin elasticity and reduce fine lines and wrinkles.
  • Mesotherapy: Mesotherapy involves the injection of a customized cocktail of vitamins, minerals, and hyaluronic acid directly into the skin. It promotes hydration, stimulates collagen production, and improves the overall texture and tone of the skin.
  • Alma DermaClear: This treatment utilizes advanced technology to provide exfoliation, deep cleansing, and intense hydration of the skin. It helps to remove dead skin cells, unclog pores, and improve skin clarity and radiance.

By incorporating these treatments into your skincare routine, you can enhance the appearance of the skin around your eyes and maintain a youthful and taut look for a longer period.
